Duplicates are caused by the fact that SSA doesn't specify the UNIQUE keyword 
on logical files that should be.  They instead count on programmers to follow 
standards to prevent duplicates, which is equivalent to counting on all 
politicians to be honest (perhaps worse).  If you are having problems with 
duplicates on a particular file and don't have a high availability system in 
place, start journaling on the files in question and do a TYPE2 dump to a 
database file when you find any duplicates.  That will point out the program, 
user, date, and time of the offending program...
