Will that work with a call to an external SQL stored procedure rather than
an RPG program call?
I think the answer is "no" but just want to be sure.

You can use PCML to describe data structures that are passed into RPG
program calls.

Something like ...

             path="/QSYS.LIB/%LIBL%.LIB/PGMORSRV.srvpgm"
             entrypoint="PROCEDURE_NAME">
             <struct name="dsName" usage="inputoutput">
                         <data name="subfield1" type="char" length="10" />
