Form Software

We are looking for form software to elimiate preprinted forms (and
eventually create bar codes).  Anyone know of any products?  Here's what
we're looking at right now:
1) AFP Utilities (which we own) and AFP Workbench for CA/95NT - can
anyone tell me what kind of PC file you need to convert to a page
segment or overlay? .bmp files don't seem to work. Also, has anyone
gotten the full version of AFP Workbench to create a segment or overlay?

2) Create!print

3) PrHelp FORMagic

4) ACOM

Any endoprsements/horror stories would be appreciated (feel free to
respond privately, if you prefer).
