Old trick:

update filename set item = '6' + substr(item, 1, 5)

P.S.  I'm braindead today, and can't remember if "+" works for string 
concatenation.

runsql 'update FILENAME set substr(item,1,1) = ''6''


I cannot get past the syntax errors with this command.
