Look for a file named Normal (or Normal.dot) of type "Microsoft Word Template". Mine is folder: C:\Documents and Settings\jdamato\Application Data\Microsoft\Templates But I'm running Windows 2000 with profiles turned on. Close Word. Copy the Normal.dot file to a safe place and delete the file from the Templates folder. When you open Word again it "should" recreate your menu correctly.
