We have the same drive, and it is in SEQ Mode, but no problems like this.
The rep we bought it from did mention when we bought it that she had seen 
numerous problems with this tape drive and had to have about half of them 
replaced that she sold due to various issues. 
If you have maintenance or it's still under warranty, I would recommend a 
service call.
