I was once on-site and watched a woman force an 8" floppy into the drive, at a slight angle, and then force the door closed. Needless to say, the floppy disk was ruined, and it took us some effort to force open the drive door and extract the mangled floppy disk from the drive.

Although she stood right there and watched our efforts to retrieve the floppy disk from the drive, she had "no idea" why the diskette was damaged, or that it was somehow her fault.
