Did you restore this lpar from another machine?
Do you have multiple lpars?
Are you using a HMC?
If you are using a HMC do NOT set your firmware to upgrade based on your
OS ptfs. If you do and you let your levels on your HMC get stagnant some
day you will run into the situation where your hmc cannot operate your
box.
But if you do not have a hmc you can refer to this

I cannot seem to find a cover letter for MH01192. And since I use HMC I
don't apply these firmware upgrades via the OS so I can't paste step by
step instructions for them.
