Probably the biggest challenge (at least in my mind) is the "Fill Out
the Form" part of your question. If it is a form that you don't
originate and you need to fill fields in the form, you have a greater
challenge than if you originate the form yourself, completing the fields
as you create the form.
iText (a Java open source project) can do this and I have some code that
wraps the iText API but it is pretty much oriented to creating a
columnar report. Changing that code to either fill fields in an
existing pdf or to create a new form wouldn't be trivial but is would be
doable.
