Personally, I would start with journaling both files. Then when the
problem occurred, I would look at the journal entries, particularly
for the item ledger file, and see if the incorrect amount was written
to start with, or if the correct amount was written and then later
changed by some program.

The journal information includes not only what job was responsible for
each record operation, but also what program.

Journaling the files does not require any changes to any programs, and
will capture the record operations regardless of what program is doing
them.

Once you know for certain what program is responsible, you can proceed
further.
