I had a conference handout up (from WMCPA) as well as the SQL reference.  The conference handout made no mention of the field issue, and I was a bit baffled b/c all fields were included prior without me specifying "ADD ALL COLUMNS."  I suppose I should have read the entire CREATE INDEX entry in the reference manual.  Your answers came back much quicker, though.
