An update:  Answers all at V7.  There are corresponding PTFs at V6.  
Nothing at V5.

The cause of the problem was an error communicating with the console 
when the system is HMC managed.  The system never made it into a 
restricted state because the console was not responding correctly.  Good 
news is there is a PTF for that:  MF56123.  Small PTF no sweat..... 
Until......

I was wrong about the pre-requisites.  It requires MF99005 to be 
permanently applied.  For those of you who don't recognize it that's 
Technology Refresh 5.  That does require an IPL.

After the PTF, with some limited testing it looks to have corrected the 
problem.  We shall see over the weekend.
