Been a busy day.
Went ahead and upgraded the firmware on our 8286-41A. This is our only
machine running spinning disks.
Ran into this problem again.

This time, instead of just an IPL I decided to call IBM. They dug around
into STRSST and whatnot and found some bus / virtual IOP had a matching
pair that was kaput. They used SST to reset it and everything started
working great. And it really sped up that PTF IPL on the guests that
already exceeding an hour and a half.
They are asking me to send in some dumps they took.
