There are a couple companies that have health care packages on Power Systems. Yes they started on AS400's but they don't make those any more :-)
My first programming job was for a healthcare company that started on S/34 with the original code written by Mel Beckman.

That company still exists and supports (sells?) a RPG based application, but their main sales focus is selling server based packages.
