Today I was prepping to update a VIOS (did you know that VIOS 4.1 is
already out?) and noticed that the LPAR had over 1000 days uptime. I
decided that a sanitary reboot would be nice and since the customer has a
Dual VIOS setup there would be no issues (and it is the secondary machine).
After a while I log into the HMC to see the LPAR stuck in CA000040 and a
bit of searching brought me to this:

Long story short, if you have a P9 machine and haven't updated the FSP
firmware in a while? beware of rebooting LPARs if the machine uptime is
above 814 days.
Luckily nothing but the redundancy has been affected and tomorrow I'll
update the firmware and VIOSes, but it can be a real head scratcher if you
have the habit of IPLing machines without doing a full IPL of the host.
