Thomas, this is a problem we are seeing more and more.
There are a few reasons this can be happening.
The conclusion I have come to is that it's not worth trying to fix SNDSMTPEMM.
We have been writing PASE code to send emails. This works flawlessly. Meanwhile, Microsoft and Google seem to be closing in on SMTP and we'll have to learn and code new protocols in the not-to-distant future. E.g.,

One intermediate tactic you can take is that if this is an Exchange server you're having problems with, find another SMTP relay and your code might work.

Subject: Emails never arrive
I apologize for bringing this up again.
I'm using SMTPSendMail (on OS v5) and the SNDSMTPEMM command (on OS v7) to
send emails with attachments from within a RPGLE program.
Reviewing the joblog indicates the emails are sent successfully (email
server engages and responds with expected messages). But emails never
arrive. This used to work a few years ago.
I've tried sending to several different domains, including personal,
business, and gmail accounts.
I've also tried this on servers that are not mine. No emails arrive.
Can someone point me in a direction to figure out why? Where else can I
look?
