I don't know where the entries came from, but I would suggest adding
more tools to your arsenal.

First, an anti-spam filter:
http://www.spamihilator.com/index2.php?lang=en is free & effective.
Second, spy-ware removal tools.  I use two as any one isn't 100%
effective.  AdAware is free from http://www.lavasoftusa.com/ and Spybot
Search & Destroy is also free from http://www.safer-networking.org/

Very recently, last few days, my email address book has 20 or 30
addresses from people i never heard of. Some were a whole group of
addresses from the local hospital, others seem random. I see no signs I
have one of the current worms.
Norton ant-virus checked every day, zone alarm blocking all day (on a
cable modem).
Any ideas how to stop addresses being added?  Can an address be added
without opening the mail? (auto open feature is off) I have not been
receiving mail from these people, but do get 100 spam a day.
