Well, if it werewn't for the fact that the good price is achieved via
rebates, I would say return a defective LCD, don't replace it.  I don't
know if you get a frsh 30 with every replacement; I would doubt it.

Dead pixels can develop at any time; they actually occur when part of
the display generation gets stuck.  But I rarely hear about it.  So
rarely that I can't even think of anyone mentioning it in the last year
or more.  Probably best to do a 72 hour burn-in on the new unit to see
if any problems occur.  Be sure to rotate images on occasion, though, as
LCDs can get image burn-in just like CRTs.
