Anyone know what might cause this error?

My gut says the tape drive is failing ... if I remove the st module and
reload it, I can access the tape drive briefly ... but subsequent
accesses give me an i/o error.

The SCSI card is an Adaptec 29160, and the tape drive is a Seagate DDS3
(I forget the exact model).
