In Midrange Computing Rob Klima  wrote such a subfile screen (about 1994). 
Perhaps the code is downloadable from www.mcpressonline.com.

I only like to ask, if this requirement is binding? Can you not do this with 
two lines of data per entry or use the SFLDROP/SFLMODE keywords (if it is a 
SFL: I think so)?
