I'm creating the first of several reports and would like for them to be 
consistent.  In the "O" specs I want to put program name, report title, page 
number, date, and time.  I've seen these things laid out in a variety of orders 
for lots of different reasons.  Thoughts on what should go where and why?  TIA.
