1 - From the online help for RPGLE available in WDSC regarding ROLBK:
Repositions the file to its position at the time of the previous COMMIT 
operation (or at the time of the file OPEN, if there has been no previous 
COMMIT operation.)
So, unless you are doing a COMMIT or ROLBK after each record it could be 
going back several rows.

2 - What makes you sure that it is hitting end of file, yet not turning on 
EOF?  Is it possible that you didn't hit End of file?  Perhaps your read 
loop has gotten into an error condition and thus
dow not %eof
  read(e) myfile
EndDo
became an infinite loop?  Maybe
dow not %eof and not %error(myfile)
  read(e) myfile
EndDo
would be better
