Opening and closing cursors is the SQL way to handle this.

A SETLL loop that took 3 minutes took more than 15 minutes using 
open/close cursor before I canceled it.  ISAM has powerful weaknesses 
too: no dynamic ORDER BY, no dynamic WHERE and no dynamic JOIN.  If you 
need one of those things [1] then ISAM won't be helpful.

[1] Of course we solved these problems in ISAM using work files and code 
generators, but I doubt this solution performs better than SQL 
open/close cursor.
