Can somebody point me in the right direction for how to include ruling

lines and boxes ...

Do you need the ruler to be to scale (i.e. 8.5" ruler with tic marks on a portrait page). What increments for the tic marks, and what unit of measure (inches, cm, etc.) for each tic mark?
