I can't answer your question as such since I have not seen this behaviour.

But the correct (or at least better) way to do this is _not_ to remove the SEP entry but rather to recompile the program and then simply use the refresh option.

My question is why would the debug server still be thinking that the
program is still being used in Service Entry Points after I removed it
from the list?
Is this a known issue with RDi 9.0.1?

Once I restarted the DEBUG sever, I was able to add the program back to
Service Entry Points.
