In Preferences > General > File Associations
In the top pane, click 'Add' on the right
Add '*.php' withoug the quotes
In the bottom pane, click 'Add' on the right
Select 'External editor'
Click 'Browse...'
Find Notepad++.exe  (on my PC it's Program Files\Notepad++\notepad++.exe)
Select notepad++ from the list (it's at the bottom)
Click OK
Click OK

Notepad++ will now be in the list of choices for a right click on a .php
file.
