This will make a very easy macro using NetData.

You will need a one line function with the INSERT INTO lib/file ...
a HTML page with the form and a page that calls the function with the
insert.

You can use this as a template:

%FUNCTION(DTW_SQL) line(in field1, in field2, in field3){
    insert into libr.file values($(field1), $(filed2), $(field3) )
%}

%html(form){
<html>
    <head><title> ....
    <form action="load" method="Put">
        <input name="filed1">...

</html>
%}

%html(load){
    @line( field1, field2, field3 )
... confimation message, etc....
%}
