[WEB400]Problem with Webfacing checkboxes at WDSC V5.1.2

We have several Webfaced projects with checkboxes, radio buttons, etc.
This was accomplished by following instructions in the IBM redbook
WebSphere Development Studio Client for iSeries V5.0.

I placed the sample javascript file in the project directory
ClientScript/usr, which would then handle the setFieldValue and
clearFieldValue functions.

Now I upgraded to WDSC 5.1.2 (Advanced Edition). The manual for version
5.1.2 has an upgraded js file (example 11-9 JavaScript functions in
myUtil.js), which I created and added to the usr directory. The
checkboxes keep giving an error when I try to check or uncheck them:

        'formId' is undefined

Can anyone help me?
