All of the things you address were added by the customer :-) I wanted
to post it as-is so I could get feedback to see if the changes were
causing the speed issue.

Here is my RPG CGI base config:
Listen *:8181

DocumentRoot /www/myrxs/htdocs

ScriptAliasMatch ^/MYRXS/(.*) /qsys.lib/MYRXS.lib/$1.pgm

<Directory /qsys.lib/MYRXS.lib>
allow from all
order allow,deny
options +ExecCGI
</Directory>

*Match directives are slower than their non-match versions but one of the things I see right away is there is a duplicate ScriptAliasMatch. The configuration files are not case sensitive so there is no need for two of them. In fact, I would get rid of the first two ScriptAliasMatch's and replace them with this:

ScriptAlias /MYLIB/RESPONSE /qsys.lib/MYLIB.lib/PGM35.pgm

Also, options +ExecCGI is redundant (but harmless).

Finally, why limit yourself to serving just .html files?

The config is fairly straight forward and is as follows:

Listen *:8181
DocumentRoot /www/MYLIB/htdocs
ErrorLog trans/error_log
LogFormat "%h %l %u %t \"%r\" %>s %b" common
CustomLog trans/access_log common
CGIConvMode %%EBCDIC/EBCDIC%%
ScriptAliasMatch ^/MYLIB/RESPONSE /qsys.lib/MYLIB.lib/PGM35.pgm
ScriptAliasMatch ^/MYLIB/response /qsys.lib/MYLIB.lib/PGM35.pgm
ScriptAliasMatch ^/MYLIB/(.*) /qsys.lib/MYLIB.lib/$1.pgm
MaxCgiJobs 50
StartCGI 20 QTMHHTP1
<Directory />
Options None
order deny,allow
deny from all
</Directory>
<Directory /www/MYLIB/htdocs>
order allow,deny
allow from all
<FilesMatch "\.html(\..+)?$">
Options +Includes
SetOutputFilter Includes
</FilesMatch>
</Directory>
<Directory /qsys.lib/MYLIB.lib>
allow from all
order allow,deny
options +ExecCGI
</Directory>
