Is XmittalBatchNum truly a number or is it just a misnomer? Unless
there is formatting over it, the failure may be an issue with casting
XX-6-20091102 to a number.

Just a WAG

'Some_Agency', '2', '0.0000', '0.0000', '0.0000', '1')

Unable to insert record: Transmittal Batch Num: XX-6-20091102

If you're getting any error messages that would be helpful. If you remove
the "or die(...)" you might get specific information on what is going on.
I'd also like to see what your query looks like when you print it out.
