Well the minimum changes to make it correct are that you need to close the Consumer_Information tag - probably should go as shown below. Also the Physical Address must either lose the space between the "words or - more likely - needs to have an underscore added as shown below. These two changes make it syntactically valid - without the schema can't say more than that.

First_Name="JOHN"
Last_Name="ISNER" > <=== Missing close tag added
<Physical_Address <=== underscore added
