The other issue with the static page, which I admit I do not understand, is this:

----------------------------------------------
<!-- OPTIONAL: include this if you want history support -->
<iframe src="javascript:''" id="__gwt_historyFrame" tabindex='-1'
style="position: absolute; width: 0; height: 0; border: 0"></iframe>

-----------------------------------------------

I do that because "'Just do it!' he explained."

I cannot imagine why anyone would disable JavaScript these days. They may as well just disconnect their device from the network/Internet :)

I am curious for the reasoning for mandating no static page?

I have dynamic pages but I still have an .html file just for these kinds
of set-up issues, and to handle a browser that does not have javascript
enabled.
