Yeah, we've had issues with that too. Do you have a cluster replica of 
this DB? We have seen where you delete the document from the home server, 
it remains in the inbox, but if you open it on the cluster server it has 
been deleted properly.  Here are some notes from my PMR:

"I ran updall -R and fixup -F on the database while I was uninstalling and 
loading Notes 7.0 for the user. The only files I kept in the c:\program 
files\lotus\* paths were her ID file and names.nsf - we started with a 
clean slate. This included getting rid of a local mail file replica. After 
opening Notes 7.0, all of the problem messages were gone (thanks to updall 
and/or fixup). I advised the user to let me know if it continues."

In some cases, I remember that after I disabled smart deletions on the 
database (this will hard delete anything that's been soft deleted!) and 
then re-enabled it, the user didn't experience this any longer.

This is on a 7.0 template, right?
