I had the same problem in a prior life.  If I recall correctly, the "Forms 
jam" message details the steps you are to take - you had to follow them 
exactly: no shortcuts.  The 400 actually issued a different "forms jam" 
message for each printer type.  I haven't had to deal with this type of 
printer in aeons - let us know if this is still the case.
