We had something like that once.  It turned out that the software had to dial 
the 9, then send a pause, then send the number of the outside line, and the 
modem had to be able to accept this, which required a hardware upgrade.  We 
talked to the vendors supplying our communications software applications & 
the vendors we got the modem from & ended up getting a special phone hand set 
that does not look like the traditional one & still it failed intermittently 
& eventually we had to have a direct phone line into the computer room.
