Jeff,

Here's the System Installation and Upgrade manual.
Memory is in appendix C.

The memory slots are labeled alphabetically and filled in pairs 
following this order (A&B, C&D, E&F, etc.)

The original memory you describe as in 1 & 3 should correspond to A & B. 
It sounds like you have the correct slots for the additional memory.


The Problem Analysis manual may have a description for the error you got.

I've found that memory may need to be reseated to work correctly.  If 
you suspect bad memory, try installing one pair (A&B) at a time.  This 
should help isolate the problem stick (DIMM).

Not sure about the rules for mixing DIMM size (16Mb - 256Mb), but I'm 
sure the pairs must match.
