917B is your MFIOP. B9xx is in the range of an IOP error, but I don't
have the SRC codes for you system. My guess is you've had a disk failure.

Kirk has some instructions to get the secondary codes that may help to
identify the specific unit (disk or tape) if it's not the MFIOP card itself.
