Sometimes conditions are not such that it will choose the index you created - maybe you're using nested views or table expressions - then the connection to the original table is lost - there will still be a recommendation but it is useless.

i've used visual explain some but have a few things that make me wonder
about it's actual effectiveness. if i run VE and create the recommended
index/view whatever then run it again it tells me that it still uses the
same access path (or rebuilds that access path) again. seems something
just ain't right there...
