I've already got a query out to Powerware technical support.

I've seen this happen PW9125 a few times, and the procedure i usually
used for debugging was the following:

Shut down UPS
Remove Relay Card
Insert Serial Card
Turn on UPS
Read error log using laptop, dump diagnostics to file
Turn off UPS
Insert Relay Card
Turn on UPS

Usually the diagnostics told the Powerware tech that they have to
replace the unit. When i just called them up with the relay card
inserted, nothing much happened, i was told to wait until it happens
again.
