"Couldn't get handle" tells you that SFTP couldn't open a file on the remote server. The word "Failure" (in this example) is what the remote server returned to your sftp client as an error message. The remote server said "Failure" as it's error message.

Why didn't it set the exit status? That I don't know, but it sounds like a bug to me.
