If you're open to commercial options, our iForms product would allow you to generate a report or form document and call in the images dynamically with almost no programming effort.
The same CL command that generates the PDF document can print it natively.

My current line of thinking is to use the itext java tools to either modify a PDF or generate a PDF from scratch. I was hoping for something easier.
