I use *DLT per an earlier suggestion you posted here some years ago.
Right now I'm re-initializing said tape with CLEAR(*YES) so that I can trash
it!

It depends.  Do you use VOLSTAT(*DLT or *KEEP)?  If you're using *DLT and
you're getting that many errors on a weekly basis - nuke it.  But if you use
one tape (foolish) every night for the last 15 years and you're using *KEEP,
your opinion may differ.
I'm also guessing, since you're running V5R1, that your tape drive isn't
exactly 'leading edge'.  IOW, used tapes for that should be a dime a dozen
(figuratively).  Just replace them.

During a Save 21, it blew off after "65 of 90 libraries processed..." with 
a
CPF5349 error.
 
As it suggested, I cleaned the tape drive, re-initialized the tape and
re-ran Save 21.  Same error occurred at the same place.  System Service
Tools (SST), too,logged an error saying that the tape was defective or the
drive needed cleaning.
 
Okay, pretty lame to use the same tape the second time, but my question is
"How do I interpret the PRTERRLOG report?"  I run this as part of our
weekend process.  This weekend's report shows:

DTO003 is the tape that crapped out.  Help for the command doesn't give 
any
clues regarding how to interpret the report, and the Info Center only 
talks
about how to run the report. 
 
So, how do I use the report to determine when to pull a tape from 
inventory?
