What about using a sound file, instead?

Actually that's relatively common. Not "substituting" but augmenting w/a
sound. It's especially important if you care about section 508 and
allowing non-visual browsers into your site.

Um, but if they're on the page then can't the bot "see" them too and
render your captcha useless? I guess you could do stuff like "What's the
third word of the fourth paragraph on this page".
