One concern I've heard expressed about SSI is that it forces the HTTP server to scan every document served, looking for <!--#include virtual="..." --> tags.

Yes and no. SSI seems like a workable solution. I'd also like to see an HTML tag implementation. Something like:
<insert id="insert" src="...">
Where the browser requests a static file to be inserted at the <insert> point. Or with JavaScript:
document.getElementByID("insert").src = "...";
